A NEW 2012 MEDICAL FORM was released in the fall of 2012.

 

The 2012 BSA medical form can be found here. Please make sure these forms are being used this summer. 2012 forms have Spanish fields.

Forms are valid for 12 months only. They must be valid through Saturday of your week at camp.

Only BSA medical forms will be accepted. School sports forms will not be accepted.

Forms completed in Spanish must be submitted by May 15 so we have time to translate.

Anyone staying at camp overnight must turn in a medical form (at least Parts A & B). Anyone staying more than 24 hours must turn in a full medical form (Parts A, B & C).
